Changing the Descriptive Data on Your Passport or Replacing a Limited Validity Passport

Submit Form DS-5504 if all the following are true:

  1. You can submit your most recent passport which has not been damaged;
  2. That passport was issued less than 1 year ago.

Completing and submitting the application

Please disregard the preprinted instructions on the online forms and submit the following by mail to the Consulate General serving your area:

  1. Completed, legible passport application (Form DS-5504);
  2. Most recent US passport;
  3. One (1) U.S. sized passport photograph. (Note: these are a different size from Australian sized passport photographs) Please refer to out list of photographers;
  4. Self-addressed, Express Post or Express Post Platinum envelope for the return of your new and old passports. We are unable to accept Registered Post envelopes to return the documents;
  5. Applicants under age 16: One parent is required to sign the application. Please submit the child’s original birth certificate and the signing parents original photographic identity, such as a valid passport;
  6. If applicable — Original documentation to support name change or change of other descriptive data (certified photocopies are not acceptable).

If you would like to submit this application in person, please book an appointment in order to do so. 

NOTE: Applicants in Australia are instructed not to submit their application directly to a passport agency in the United States. The passport agency will not accept the application.
