Renewing your passport and/or amending your legal name on your passport

Since September 26, 2005, Passport Services no longer amends currently valid passports. Instead, customers are issued replacement passports.

To apply by mail you MUST answer YES to ALL four requirements.

  1. You can submit your most recent passport which has not been damaged;
  2. You were at least 16 years old when that passport was issued;
  3. That passport was issued more than 1 year ago;
  4. That passport was issued less than 15 years ago.

Please follow the instructions below and click on the links to download the relevant forms. Please disregard instructions and addresses on the online forms — these are not applicable when applying outside of the U.S.

Completing the application

Please type or print legibly in black ink when completing all sections of the application. Please print out and submit only one-sided pages of the application.

Please submit the following by mail to the Consulate General serving your area:

  1. Fully completed passport application (Form DS-82). Please see notes on downloading forms.
  2. Previous US passport (not damaged; issued more than 1 year and less than 15 years ago; issued after applicant was 16 years of age)
  3. Two (2) passport photographs. 5cm × 5cm. (Click here for a list of photographers)
  4. If applicable — documentation to support name change or change of other descriptive data (please read important information regarding supporting documents)
  5. Applicable Fee* (All fees are subject to change without notice)
  6. Pre-paid self-addressed Express Post Platinum or Registered Post envelope for return of your passports.

* If you are applying for a new passport to correct an error in your identifying data (e.g. spelling of name, date or place of birth) and the error was caused be a printing error by Passport Services, there will be no fee. You will be issued a new passport with a new number and a new issue date, but with the same expiration date of the replaced passport.

Please disregard the instructions and addresses listed on the online version of the form and submit it via mail to the Consulate General serving your area.

Please do not submit your application directly to the United States. Doing so will delay processing of your application.
