Passports for applicants 16-17 years of age

If you are applying for the first passport for a child under the age of 18, who was born outside of the U.S., you must first report the child's birth. Please see Consular Report of Birth Abroad.

The applicant (child) must appear in person.

If your child does not have current identification of his/her own, a parent will need to accompany the child and present identification.

Please follow the instructions below and click on the links to download the relevant forms.

Please disregard the instructions and addresses listed on the online version of the forms and submit your application in person with your child at the Consulate General serving your area. You will be required to make an appointment.

Completing the application

Please print out and submit only one-sided pages of the application. Please type or print legibly in black ink.

Please complete all sections EXCEPT Section 23. You need to sign this in front of the Consular Officer.

Please be sure to complete all forms, and bring all required supporting documents and evidence in original format.

To be presented in person at the Consulate:

  1. Completed (but not signed) passport application (Form DS-11). Please see notes on downloading forms.
  2. Applicant (child)
  3. Evidence of child’s U.S. Citizenship*
  4. Applicant’s current evidence of identity*
  5. Parents’/guardians’ evidence of identity*
  6. Evidence of child’s relationship to parents/guardians*
  7. Two (2) passport photographs. 5cm × 5cm. (See our list of photographers)
  8. Applicable fee (All fees are subject to change without notice)
  9. (Optional) Pre-paid self-addressed Express Post Platinum or Registered Post envelope for return of your passports. If you do not provide an envelope, you must collect your passport in person from the Consulate General at which you applied.

If applicant does not have valid photo identification, application must also include parental/guardian consent. If parent/guardian cannot be present, please bring notarized consent form (DS-3053) and non-attending parent’s/guardian’s proof of identity. Helpful hints for applications made via the Sydney Consulate:

  • Make a copy of all supporting documents (not the application forms) to leave with us. If photocopies are not provided, you will need to pay $1 USD for each photocopy (if you are also applying for a Social Security Number, please make two (2) copies of each supporting document);

Photo Guidelines

Two identical color photographs required 50mm × 50mm (2" × 2"); headsize chin to top of head minimum 25mm (1"), maximum 35mm (1 3/8").

Please read the Department of State Composition Checklist, or refer to our list of photographers known to the U.S. Embassy who take U.S. sized passport photographs.
