Additional Visa Pages

Additional visa pages will not be added to an expired, damaged or mutilated passport, or to passports that were issued with 1 year or less validity. No more than two sets of visa pages will be added to any one passport.

Completing the application

Please disregard the preprinted instructions on the online forms and submit the following by mail to the Consulate General serving your area:

  1. Self-addressed Express Post or Express Post Platinum envelope for return of your new and old passports. Please note that we are unable to accept Registered Post envelopes to return the documents;
  2. Fully completed, legible and signed application form (Form DS-4085); 
  3.  Current U.S. Passport;
  4. Applicable fee* (All fees are subject to change without notice);
  5. Applicants under age 16: One parent is required to sign the application. Please submit the child’s original birth certificate and the signing parents original photographic identity such as a valid passport.

 

If you would like to submit this application in person, please book an appointment in order to do so.  While we are generally able to add the additional visa pages while you wait, there may be times when you are requested to return later in the day to collect your passport, or to provide an envelope (see item 1).  If you choose to submit this application in person, you should allow at least one hour at the Consulate. 

NOTE: Applicants in Australia are instructed not to submit your application directly to a passport agency in the United States. The passport agency will not accept the application.
